Use fictional answers only in this local prototype. Assessment answers are held only in temporary browser memory. There is no account, database, analytics integration, email collection, or answer-submission endpoint.

Restarting or reloading clears the assessment. Browser back/forward behavior may retain a live page in memory until it is closed. Do not enter names, addresses, medication names, or real health details.

Printing or saving a PDF creates a copy under your control, outside this application. Optional state, ZIP, payment, and housing fields are also temporary.

The local web server serves application files. No assessment answers are placed in URLs, cookies, browser storage, or server requests by the application. External educational links open their providers’ sites under their privacy practices.

Before public launch, a qualified reviewer must finalize the privacy policy, verify hosting and logging practices, and approve any new data flow.
